If you would like to register with St.Katharine Docks practice, please scroll down and read section “Am I registering with the right practice?”
Please complete this registration form and email this to us together with “proof of address and personal photo ID” at nelondonicb.skd@nhs.net. Please see what “proof of address and personal ID” required. Proof of address must be current – within the last 12 weeks.
If you have been previously registered with an NHS GP practice please provide us with your NHS number on the registration form – this will enable us to obtain your previous medical records quickly. We will return incomplete form back to you – so please make sure you read the form and answer the questions carefully. Please also make sure we have your personal mobile number and your email address.
If you do not have access to internet please collect the registration form from the practice and complete it at home.

Am I registering with the right practice?
Please note that you must live within the practice boundary to be able to register with us. In order to register, you must live in one of the following areas:
Burr Close, Cloysters Green, Ivory House, Jacana Court, Marble Quay, Maudlin’s Green, Merganser Court, Mews Street, Millers Wharf, Nightingale House, Osprey Court, President's Quay House, Riviera Court, Sandpiper Court, Sanderling Lodge, Shearwater Court, Star Place, St. Anthony's Close, St.Katharines Dock, St.Katharines Way, Stockholm Way, Swan Court, Teal Court, Tower Bridge Wharf, Tower Bridge Walk, Tower Hotel Staff Hostel, Mathilda House.
If you do not live within our practice boundary please visit www.nhs.uk, for more information about how to find a GP or Dentist. If you have an urgent medical problem which needs urgent attention please dial 111 service.
