How do I get my test results?

You can obtain your test results by:

  • Using the NHS App (where available)
  • Contacting the practice by telephone
  • Speaking to a member of our reception team

Please note that some test results require review by a GP before they can be released. If further action is required, a member of the clinical team may contact you.

How do I request a sick note (Fit Note)?

If you have been unwell and require a Fit Note, please contact the practice.

You can request a Fit Note by:

  • Completing an online consultation
  • Contacting the practice by telephone
  • Speaking to a member of our reception team
  • Send us an email at nelonconicb.skd@nhs.net

Please note:

  • A Fit Note is usually not required for the first 7 calendar days of sickness absence.
  • You can self-certify for the first 7 days using your employer's self-certification process.

How do I register as a patient?

We welcome new patients living within our practice boundary.

To register:

  • Complete our online registration form (if available)
  • Download and complete a registration form from our website
  • Visit reception and request a registration form

You do not need proof of address, identification or an NHS number to register with a GP, although these documents may help us locate your medical records more quickly.

How do I change my address or personal details?

If you have changed your:

  • Address
  • Telephone number
  • Mobile number
  • Email address
  • Name

Please inform the practice as soon as possible.

You can update your details by:

  • Completing an online form
  • Visiting reception
  • Contacting us by telephone

Keeping your contact details up to date helps ensure you receive important information about your healthcare.

How do I order repeat prescriptions?

Repeat prescriptions can be requested in several ways:

  • Through the NHS App
  • Through Patient Access
  • Using your usual online service
  • By submitting your repeat prescription request to the practice
  • Send us an email at nelonconicb.skd@nhs.net

Please allow sufficient time (at lease 48 hours) for your prescription request to be processed.

For patient safety, prescriptions cannot always be issued immediately and may require review by a clinician.

How do I access my medical records?

You can request access to your GP medical record through:

  • The NHS App
  • NHS-approved online services
  • A written request to the practice

Depending on your circumstances, you may be able to view parts of your medical record online, including:

  • Test results
  • Medications
  • Allergies
  • Vaccinations
  • Consultation history

If you need help setting up online access, please contact the practice.

How do I book an appointment?

Appointments can be requested by:

  • Online consultation services
  • The NHS App (where available)
  • Visiting reception

Our team will help direct you to the most appropriate service for your needs.

How do I request a home visit?

Home visits are available for patients who are unable to leave their home because of their medical condition.

If possible, please contact the practice before 11.00am to request a home visit.

A clinician will assess the information provided and determine the most appropriate course of action.

Do you provide interpreter services?

Yes.

If English is not your first language, we can arrange a professional interpreter free of charge.

Please let us know when booking your appointment if you require interpreter support.

We can also discuss other communication needs and reasonable adjustments where appropriate.

How do I request support or reasonable adjustments?

Please tell us if you need:

  • Large-print information
  • Easy Read information
  • British Sign Language support
  • Assistance accessing our services

Our team will discuss your needs and consider reasonable adjustments to help you access healthcare.

How do I make a complaint or provide feedback?

We welcome patient feedback and use it to improve our services.
