Support for Vulnerable Patients
We are committed to supporting patients who may be vulnerable due to:

Supporting Homeless Patients
Everyone has the right to register with a GP.
If you are homeless or do not have a fixed address, you may still be able to register with our practice if you fall within our practice boundary.
You do not need proof of address, immigration status, identification or an NHS number to register with a GP.
Our team can provide advice and support to help you access healthcare services and local community resources.
Translation and Communication Support
We understand that language and communication barriers can make it more difficult to access healthcare.
We can arrange support including:
- Professional interpreters
- British Sign Language (BSL) interpreters where available
- Large print information
- Easy Read information
- Communication support for patients with additional needs

Social Prescribing
Sometimes practical, emotional or social issues can affect health and wellbeing.
Social Prescribing helps connect people with community services and local organisations that may be able to provide additional support.
Patients may be signposted to services that support:
- Mental wellbeing
- Social isolation and loneliness
- Physical activity and exercise
- Welfare and benefits advice
- Housing support
- Community activities
- Carer support
- Volunteering opportunities

Mental Health Support
Mental health is just as important as physical health.
If you are experiencing difficulties with your mental wellbeing, support is available.
You may wish to speak to a GP if you are experiencing:
- Stress
- Anxiety
- Low mood

- Bereavement
- Loneliness
- Relationship difficulties
- Work-related pressures
Urgent Mental Health Support
If you need urgent mental health support, you can contact NHS 111 and select the mental health option, available 24 hours a day.
If there is an immediate risk to life, call 999 or attend your nearest Accident and Emergency Department.

There are many organisations within Tower Hamlets and North East London that provide valuable support to residents.
Community services may include:
